About the Role

As a Fractional Chief Medical Officer or Chief Clinical Officer, you will serve as a senior clinical advisor to Siftwell Analytics, helping ensure the company’s analytics-driven insights are clinically credible, operationally realistic, and trusted by healthcare customers.

Reporting to the CEO and working closely with Product and Customer Success teams, you will translate population health and care management insights into practical clinical context that supports customer understanding, adoption, and confidence in Siftwell’s platform. You will act as a clinically-fluent thought partner, shaping how insights are framed, discussed, and applied within real-world care delivery and managed care operations.

This role operates in a purely advisory and enablement capacity. You will not provide clinical decision-making, direct patient care, or regulated medical services. Instead, you will support internal teams and, on a limited basis, customer-facing discussions by grounding analytics outputs in established clinical workflows, care management practices, and operational constraints.

When appropriate, you may be positioned externally as a Siftwell Clinical Advisor, reinforcing clinical credibility while customers retain full ownership of all clinical and operational decisions.

This role is ideal for a senior physician or clinical executive with deep experience in care management, population health, or payor-provider operations who enjoys working at inflection points and influencing impact without owning day-to-day execution.

Terms of Engagement

  • Provide up to approximately ten (10) hours per month of advisory support during the initial six-month engagement term.

  • Allocate time flexibly across internal preparation, customer-facing sessions (as needed), and follow-up discussions.

  • Monthly hours may flex based on active customer needs and Siftwell requirements.

  • Operate with short planning horizons (often one week or less), coordinating preparation and scheduling collaboratively.

  • Work exclusively with summary-level, de-identified information.

  • Operate downstream of Siftwell's platform outputs, focusing on interpretation and operational context rather than analytic development or validation.

Responsibilities

  • Collaborate with Siftwell in an iterative, consultative manner that emphasizes preparation, discussion, and applied guidance rather than formal deliverables.

  • Support work driven by active customer use cases and internal priorities.

  • Participate in internal preparation sessions to review upcoming customer discussions, cohort outputs, and presentation materials.

  • Lead focused working sessions to interpret analytics outputs from a clinical and care management operations perspective.

  • Participate selectively in customer meetings to support interpretation, intervention discussion, and workflow alignment.

  • Conduct follow-up discussions to refine messaging, approach, and next steps.

  • Interpret Siftwell-generated cohorts and population insights in clinically meaningful, non-prescriptive terms.

  • Help identify likely clinical and operational drivers of risk or utilization.

  • Help differentiate impactable populations from maintenance or monitoring populations.

  • Advise on appropriate types of interventions and escalation pathways.

  • Discuss common engagement barriers and operational considerations.

  • Help customers assess staffing models, ratios, and existing programs in context.

  • Guide structured discussions using Siftwell’s data, Pegasus platform outputs, and clinical domain expertise.

  • Help surface tradeoffs and considerations without directing clinical decisions.

  • Reinforce that customers retain final authority over all clinical and operational actions.

  • Review slides, summaries, and materials ahead of key customer sessions.

  • Provide contextual guidance to Siftwell teams to improve clinical fluency over time.

  • Offer feedback to Siftwell’s Product team on how customer clinical input may inform future product enhancements.

Ideal Background & Experience

  • Active clinical licensure (e.g., MD, RN or equivalent).

  • Deep managed care experience, with direct exposure to payer environments such as Medicare, Medicaid, or commercial health plans.

  • Senior clinical executive experience at the health plan or enterprise level, serving in a Chief Medical Officer, Chief Nursing Officer or similar clinical executive role with organization-wide accountability (not state- or region-limited roles).

  • Physician or clinical executive background, with demonstrated leadership of corporate clinical initiatives across multiple functions or programs.

  • Strong quality and performance measurement experience, including hands-on familiarity with HEDIS and related quality, utilization, or outcomes measures used in managed care environments.

  • Experience translating quality and population health metrics into operational programs, care management strategies, and executive-level decision support.
