Field Case Manager

This 12-month contract role offers an opportunity to make a meaningful difference in patients’ healthcare journeys.
You will serve as a trusted point of contact for healthcare providers, patients, and program partners within your designated region.
Working primarily from a dedicated home office, you will also visit healthcare professionals and clinics to build strong in-person relationships.
You will help patients and providers navigate therapy access, reimbursement, enrollment, and authorization processes.
The role combines clinical knowledge, patient advocacy, organization, and relationship management in a dynamic healthcare environment.
You will take ownership of complex processes while identifying opportunities to improve service quality and patient support.
This is an autonomous and flexible position suited to a healthcare professional who is motivated by patient impact and collaborative problem-solving.
Accountabilities:

Serve as the primary contact for physicians and healthcare provider teams within the assigned territory.

Communicate with patients and program stakeholders as required, developing and maintaining trusted professional relationships.

Educate healthcare providers and their staff about program services, patient support resources, therapy navigation, reimbursement, and enrollment processes.

Review patient charts and ensure all required documentation is accurate and complete, including special authorization forms, enrollment documentation, and prescription renewals.

Take ownership of the special authorization process, ensuring submissions are timely, accurate, and appropriately supported to facilitate reimbursement.

Provide physicians with ongoing updates and guidance regarding patient status, program changes, processes, and relevant best practices.

Adapt program services to meet the specific needs of healthcare providers and patients across both in-clinic and remote settings.

Identify challenges and proactively develop solutions that improve the quality and effectiveness of patient support.

Collaborate closely with program colleagues and healthcare professionals to coordinate activities and deliver a consistent patient-centered experience.

Participate in healthcare education events with healthcare professionals when required, including occasional events outside standard business hours.

Follow established program performance standards, procedures, privacy requirements, and operational guidelines.
Requirements:

Registered Nurse (RN), Bachelor of Science in Pharmacy, another relevant science degree, or equivalent transferable healthcare experience.

Knowledge of Canadian public and/or private third-party reimbursement environments.

Previous case management experience is an asset.

Bilingual English and French communication skills, both written and verbal, are required for positions supporting Quebec and New Brunswick.

Strong understanding of medical terminology and the ability to communicate confidently with physicians and healthcare teams.

Exceptional interpersonal and customer-service skills, with the ability to establish credibility and build trusted professional relationships.

Strong computer literacy and proficiency with Microsoft Office applications.

Ability to exercise sound judgment, demonstrate a sense of urgency, and maintain accuracy when handling sensitive patient and healthcare information.

Valid, unrestricted, non-expired driver's license and access to a personal vehicle.

Ability to attend required healthcare education events, including occasional activities outside regular business hours.

Ability to maintain a private, dedicated home-office space suitable for company-provided equipment.
